Key Takeaways
- In the U.S., the Luvox (fluvoxamine) labels list OCD as the approved use. Adults start at 50 mg at bedtime on tablets or 100 mg on Luvox CR, rise in 50 mg steps, and can reach 300 mg a day.
- A meta-analysis of fixed-dose OCD trials found high SSRI doses reduced symptoms more than low or medium doses, a pattern that differs from depression, though higher doses also caused more side-effect dropouts.
- Using fluvoxamine for anxiety is off-label. Social anxiety trials used a range similar to OCD, and a large meta-analysis found higher SSRI doses helped anxiety disorders too.
- Fluvoxamine levels rise faster than the dose, and the drug slows the breakdown of medications like alprazolam, theophylline, and warfarin, so age, sex, liver health, smoking, and other prescriptions all shape the right dose.
Maybe you started Luvox at 50 mg for OCD, and your prescriber keeps nudging the dose up. Maybe you read that someone takes it for social anxiety and wonder whether the same numbers apply to you. The same pill gets dosed differently depending on what it’s treating, and the reasons are specific.
Fluvoxamine, the drug in Luvox, is an SSRI, and the Luvox CR label says its effect in OCD is presumed to come from blocking serotonin reuptake in brain neurons. How much blocking is enough seems to depend on the condition. Dosing Luvox with precision starts with knowing which condition you’re treating.
The usual SSRI approach is to start low, raise the dose gradually, and stop at the dose that works, and for depression it’s well supported. A 2019 depression meta-analysis of 77 fixed-dose trials found SSRI benefit rose up to 20 to 40 mg in fluoxetine equivalents and then leveled off, while dropouts from side effects kept climbing. The results are real, and the science is well established.
But the depression playbook tends to undershoot OCD, and at Siggy we don’t think your dose plan should borrow its rules from a different condition. Condition-matched dosing sets the target and the timeline by what you’re treating: higher and further for OCD, guided by trial ranges for anxiety, and adjusted for everything else you take. It answers the same question, which dose fits you, with your condition’s evidence in view. The right Luvox dose depends on the condition it’s treating, and that difference comes straight from the evidence.

Luvox Dosage at a Glance
The fluvoxamine tablet label and the Luvox CR label set the OCD starting doses, steps, and limits below, and the last row shows how one social anxiety trial dosed it:
| Use | Starting Dose | Increase | Daily Maximum |
|---|---|---|---|
| Adult OCD, tablets | 50 mg at bedtime | 50 mg every 4 to 7 days | 300 mg |
| Adult OCD, CR capsules | 100 mg at bedtime | 50 mg weekly | 300 mg |
| OCD, ages 8 to 11 | 25 mg at bedtime | 25 mg every 4 to 7 days | 200 mg |
| OCD, ages 12 to 17 | 25 mg at bedtime | 25 mg every 4 to 7 days | 300 mg |
| Social anxiety, off-label | 50 mg | 50 mg weekly | 150 or 300 mg studied |
The social anxiety row comes from a 10-week Japanese trial, in which fluvoxamine started at 50 mg a day and rose 50 mg a week to a cap of 150 or 300 mg. On tablets, the label advises splitting daily totals above 100 mg into two doses, with the larger one at bedtime, and for children the split starts above 50 mg.
Why OCD Doses Run Higher
For OCD, the evidence that higher SSRI doses help is direct. A fixed-dose OCD meta-analysis pooled 9 trials with 2,268 adults and found high doses reduced symptoms more than low or medium doses and produced more responders. The authors estimated that for every 13 to 15 people treated at a high rather than lower dose, 1 more would respond.
The cost was tolerability: about 1 in 17 people treated at a high rather than low dose dropped out because of side effects, though dropouts for any reason didn’t rise. Only one of the nine trials tested fluvoxamine, so this is evidence about SSRIs as a class.
The authors contrasted this pattern with depression, where a meta-analysis they cited found no added benefit from higher doses. Anxiety disorders show a dose effect too: a 57-trial anxiety meta-analysis of 16,056 adults found higher SSRI doses brought more improvement and more responders, along with more side-effect dropouts. Still, the OCD authors noted that many experts push SSRI doses higher and faster for OCD than for other anxiety disorders or depression.
How Luvox Is Titrated for OCD
For OCD, the tablet label says to keep adding 50 mg steps as tolerated until you reach the most benefit, without passing 300 mg. In the two adult tablet trials behind the approval, the CR label says people reached 150 mg within 2 weeks, then adjusted within 100 to 300 mg, split into two daily doses. In the 12-week Luvox CR trial, the average final dose was 261 mg.
The steps are spaced for a reason. The CR label reports that tripling the dose from 100 to 300 mg raised blood levels about 5.7-fold, with steady levels reached within about a week. Each increase can land harder than its size suggests.
Higher targets don’t mean waiting months to feel anything. A 2016 OCD meta-analysis of 17 trials and 3,276 adults found SSRIs separated from placebo within 2 weeks, with the biggest gains early and greater response at higher doses. Patience still matters, because treatment-resistance definitions cited in the dose-response meta-analysis require no improvement on multiple SSRIs at the highest tolerated dose for at least 2 months.
Once it works, the label says OCD generally needs several months or longer of treatment, at the lowest effective dose, reviewed periodically. Tablets come in 25, 50, and 100 mg, with the 50 mg and 100 mg scored, and CR capsules come in 100 and 150 mg and shouldn’t be crushed or chewed. The dose-response meta-analysis describes CBT and SSRIs as first-line OCD treatments, and our guide to exposure therapy for OCD and anxiety covers the therapy side.
How Luvox Is Dosed for Anxiety Off-Label
In the U.S., the fluvoxamine tablet label and the Luvox CR label list OCD as the approved use, so prescribing it for anxiety is off-label. The dose plan then comes from clinical trials and your prescriber’s judgment rather than a label target.
Those trials used a range much like OCD. In a 12-week U.S. trial of 92 people with social anxiety, the fluvoxamine group averaged 202 mg a day, and 42.9% responded compared with 22.7% on placebo. The Japanese trial also beat placebo, and its most common side effects were sleepiness in 45.1% of people and nausea in 23.1%.
Fluvoxamine has been studied in young people with anxiety, too. A pediatric anxiety trial found it effective for children and teens with social phobia, separation anxiety disorder, or generalized anxiety disorder. The label’s boxed warning covers suicidal thinking in children, teens, and young adults taking antidepressants, and it states fluvoxamine isn’t approved for pediatric use except for OCD.
So anxiety dosing isn’t automatically low. For the same reasoning applied to a different SSRI, see our guide to how doctors decide on a Lexapro dose.
Who Needs a Different Luvox Dose
According to the labels, several groups clear fluvoxamine differently or reach higher levels at the same dose:
- Children ages 6 to 11 had steady-state levels 2 to 3 times higher than teens, and the label caps doses at 200 mg through age 11.
- In exploratory analyses of the pediatric OCD trial, the benefit showed up mainly in 8 to 11 year olds, and the label suggests lower drug exposure in teens may explain it, so teens may need up to the adult maximum of 300 mg.
- In a CR study, women had about 60% higher levels than men at the same 100 mg dose, and among children, girls had higher levels than boys, so girls may respond to lower doses.
- Older adults clear fluvoxamine at about half the rate of younger adults, so a lower start and slower steps are recommended.
- With liver problems, clearance is about 30% lower, and the label advises starting low and increasing slowly.
- Smokers metabolize fluvoxamine about 25% faster than nonsmokers, so tell your prescriber if you start or stop smoking.

Drug Interactions That Change the Plan
Fluvoxamine slows several liver enzymes that clear other drugs. The label describes it as a potent inhibitor of CYP1A2 that also inhibits CYP2C9, CYP3A4, and CYP2C19, so your Luvox dose can change the dose you need of something else.
Some combinations are off the table. The labels list tizanidine, thioridazine, pimozide, and alosetron as contraindicated, the CR label adds ramelteon, and MAOIs need a 14-day gap on either side. In one study, fluvoxamine raised tizanidine exposure about 33-fold.
Other drugs need a lower dose or closer monitoring:
- Alprazolam levels roughly doubled with 100 mg of fluvoxamine, so the label says to at least halve the starting alprazolam dose.
- Diazepam is generally not advisable, because both it and its active byproduct can build up.
- Theophylline clearance fell about threefold, so its dose should drop to one-third, with blood levels checked.
- Warfarin levels rose 98%, so clotting times need monitoring and the warfarin dose adjusted.
- Propranolol and metoprolol need lower starting doses and slower increases.
The label notes some of these interactions may be stronger at higher fluvoxamine doses, which matters for OCD, where the CR trial’s average final dose was 261 mg.
With a medication that climbs in steps, each step raises new questions between visits about sleep, nausea, and whether symptoms are moving.
If OCD is your main concern, working with a therapist trained in exposure and response prevention is a core part of treatment.
Luvox dosing follows the condition. For OCD, it climbs in 50 mg steps within a 100 to 300 mg range, trading more benefit for more side effects as it goes. For anxiety, it’s off-label and guided by trials that used a similar range, and your age, sex, liver, smoking, and other medications can shift the numbers either way. Frequently Asked Questions
What is the usual starting dose of Luvox?
Adults with OCD usually start fluvoxamine tablets at 50 mg at bedtime, or Luvox CR capsules at 100 mg at bedtime. Children and teens ages 8 to 17 start tablets at 25 mg at bedtime. The dose then rises gradually based on response and side effects.
What is the maximum dose of Luvox?
The label maximum is 300 mg a day for adults and teens ages 12 to 17, and 200 mg a day for children ages 8 to 11. On tablets, the label advises splitting totals above 100 mg into two doses, with the larger dose at bedtime.
Is the Luvox dose for anxiety lower than for OCD?
Anxiety doses aren't always lower. Fluvoxamine's U.S. labels list only OCD, so anxiety dosing is off-label and based on trials, and a U.S. social anxiety trial averaged 202 mg a day. The authors of an OCD dose-response meta-analysis noted that many experts push SSRI doses higher and faster for OCD than for other anxiety disorders.
How long does Luvox take to work for OCD?
SSRIs like Luvox can start easing OCD symptoms within 2 weeks, but a full trial takes longer. In a meta-analysis of 17 OCD trials, SSRIs beat placebo within 2 weeks, with the largest gains coming early. Treatment-resistance definitions call for at least 2 months at the highest tolerated dose before a medication is judged not to work, and the label says OCD generally needs several months or longer of treatment.
Can you take Luvox once a day?
You can take fluvoxamine tablets once a day at bedtime at doses up to 100 mg. Above that, the label advises two doses a day with the larger one at bedtime. Luvox CR capsules are taken once a day at bedtime and shouldn't be crushed or chewed.
What drugs should not be taken with Luvox?
The labels list tizanidine, thioridazine, pimozide, alosetron, and MAOIs as off-limits with fluvoxamine, and ramelteon shouldn't be combined with it either. Fluvoxamine also raises levels of alprazolam, diazepam, theophylline, warfarin, and some beta-blockers, so those need dose changes, extra monitoring, or an alternative. Tell your prescriber about every medication and supplement you take.
